How would we treat reflux with the benefit of hindsight

Hi,
I have been watching The "interesting" thread. Haven't dared read the articles as have enough to worry about without Omeprazole being added to the list at the moment. I am an ostrich I know!
But it set me thinking, given what you all know now, where would you start with tackling another refluxer. I am intrested more than anything and it may be a useful thread for anyone new to the site.... Never know if we get some good responses the adminstrators may be able to do something more useful with it still.
I'll kick it off below, but please do add on!

How I would tackle another refluxer... Working down the list giving each time to work before heading for the next,
Get checked for Tongue tie by someone knowledgable (milk matters website)
Give a probiotic
Exclude dairy and soya from my diet
put on to neocate formula, thrugh a dr browns bottle, thickened with Carobel
Add in a ppi - I would push for omperazole without rantitine in the hope of healing the oesophegus and being able to wean off it again.
If vomiting is uncontrollable try domperidone.
Tests...
Referral to a gastro pead as early on as possible.

I'm the same Sarah, I've opened all the links, scanned the first lines then closed them again. Having just has Lola off Omep for 10 days of hell I can't consider it not being right to put her back on. Anywho, great idea. Here is mine:

Practice attachment parent from the get go. Co sleep, safely of course! I might buy a side car cot next time but I certainly won't be bothering with forcing it into a crib like I did with Lola, even if doesn't have reflux. Use a sling and not let anyone scare me with their silly talk of rods and backs, routines, schedules and manipulation blah blah blah. I would basically give her no reason to cry, then I would know if she was crying for a reason! IYSWIM.

Check for TT with someone MEGA qualified, and get proper BF help.

Sack my HV

Breastfeed for longer and cut out dairy and soya

Go straight to Neocate if I did stop BF

In the early screaming days, have more help and prepare more (sleep more in day, have more precooked meals)

Try meds. I would try Gaviscon again but don't think I would bother with Ranitidine again

Demand a gastro paed, and do tests if 20 mg Omep is not helping or has other symptoms. I don't think I would have another PH study unless it was at the same time as scopes, but I might change my mind on that when we get our results, and I get over the trama.

Not stress about early weaning AT ALL, and do baby led from 6 months

Believe in my myself more and trust my intuition. Do what I feel is right, not what I think is right. Thinks thats all!

I would do the same as you lot.

Def check for a tt at birth

Breastfeed for longer ignoring all advice not to. With Cole being a prem i was very careful about what i ate more so than the first time round. Meat, fish and veg. Fruit to snack. Also drinking fennel and raspberry leaf tea. No soya and only a small amount of dairy. (prob why he never had reflux on breastmilk ).

Straight to Neocate if this didn't work.

Prebiotics

Referal to gastro and demand Scopes, barium and ph.

Despite what i've read if the tests show problems start omeprazole.

Weaning following the MEWS diet, gradually introducing one thing at a time waiting at least 4 days before trying something else.

 

And finally as Susie said trust mothers instinct. Fight for what you want and dont let dr's talk me into anything else.

Hi Teresa, sorry if this is a silly question but what are scopes, ph and barium? My little one is 17 months old now and has awful silent reflux and only copes with a very limited diet, pepti milk, omeperazole, domperidone and CMP,soya and egg avoidance and she is still dtruggling. To make matters worse the dietician and paed at the hospital want her off everything by the time shes two- whilst i dont like her being on all the meds i know she needs them because if i miss a dose we have HELL! I just wondered if the scope/phs etc was a medical test or something i can request to see why things are still bad for my little one, having a nitemare as she cant seem to manage to eat anything without being uncomfortable. :(

I agree wholeheartedly with all of the above!

I'm on my second refluxer... and followed all the ideas Sarah B has posted! The only thing I will do differently if ever mad enough to try for a third is to go dairy-free throughout my pregnancy!(been under gastro-paed team from 2 days of age and on omeprazole since 3 days; dairy, soya and wheat free from 5 weeks - but we have a family history of dairy intolerance so with hindsight probably should have been dairy free from day dot!)!

I would add in "speak to homestart" and get a volunteer for  hours a week just to look after baby so you can get much-needed sleep.

Hmm, I have been giving this a considerable amount of thought recently. At first I was adamant I didn't want to got through all this again, but then caught a few episodes of One Born Every Minute USA that I had recorded, and found myself feeling clucky! Anyway, not that I'm sure we are totally there yet, here's what I would do.

Check for tongue tie with a private lactation consultant ASAP after birth

Get LO checked over by a paediatric qualified osteopath

I'm really not sure on the breastfeeding after having such a horrible experience this time (probably down to the TT), do I want to go through all of that again? Having said that, that's a bit selfish on my part, breast milk is the best thing in any case, but definitely for a refluxer. Would go dairy and soya free too. Absolutely no formula top up's though.

If BF doesn't happen, go on to Neocate.

I probably would try rinitidine again, but not domperidone unless LO is really sicky. Didn't seem to do much for my LO's silent reflux. Give it two weeks and be patient, if that doesn't do the job, straight to omeprazole.

Probiotics

Good idea Susie Q, although i did prepare some meals, I could have done with loads more. Allow my mum to look after LO a lot more so I can sleep.

Space visitors out a bit better and not feel pressured.

Can't comment on the tests part, as we have not gone through that (yet), but I guess if things don't settle I would push for them.

Feel more confident in myself and the decisions I have made, and not doubt myself AT ALL despite what family and friends say are normal.

I've been thinking about this too, although I'm not sure if we'll go for a third. If you'd asked me when Matthew was 5 or 6m old I'd have said "never again" but now there is light at the end of the tunnel the idea seems more attractive.

 

Restrict dairy products throughout pregnancy (dairy was one of the things I craved and I ate loads of, the book I'm currently reading about food intolerances suggests this may be one thing that can trigger an intolerance, and I wonder if this was a factor in Matthew's low birthweight - OMG I was poisoning him in the womb....)

Go dairy free completely from 38 weeks so breastmilk should be dairy free from day 1.

Try to avoid any antibiotics in pregnancy or post-partum (I had them for 2 weeks post partum because I had surgery after the birth, but I didn't have an infection, they were precautionary, and I now wish I hadn't taken them)

Get checked for TT immediately and corrected ASAP

Breastfeed dairy and soya free initially and introduce them slowly to see if there is any reaction. Absolutely no formula.

If baby was a refluxer, not bother with gaviscon, start on ranitidine ASAP and go onto omeprazole much sooner.

Trust myself more and feel confident in my decisions. Not feel the need to justify myself to interfering family members (who STILL don't believe Matthew is CMPI, I just do all this for my own amusement)

I think most of it is covered by you lovely ladies but some more.


No formula under any circumstances (mw gave SMA gold to my LO on 2nd day in the hospital).

No antibiotics unless absolutely needed.

No early weaning specially no baby rice or jar foods.

Dairy and soy free breast feeding from starting and if possible same during pregnancy.

Don't take 'he/she will grow out of it' from doctors.

Don't accept colic as a reason for anything and everything.
