Daniel’s follow up appointment with paediatrician

Daniel saw the paediatrician last Friday afternoon, last time he saw him was the end of February (good old NHS) and he was put on omeprazole suspension (10mg/5ml) one 2.5ml dose a day (paediatrician gave us a 2 month prescription) and domperidone 5mg/5ml one 2.5ml dose three times a day. When I went to pick up a repeat prescription from GP they had prescribed Omeprazole 5mg/5ml one 5ml dose a day which I queried with paediatrician and he said it was ok just a different concentration. I basically said that although he has improved, he doesn’t vomit like he used to (5-10 times a day previously) (now down to about 2-5 times a week) and doesn’t scream constantly, he is still a poor sleeper, difficult to settle and wakes constantly. The paediatrician has increased his dosage of domperidone (5mg/5ml, a 6ml dose three times a day) but left the omeprazole dosage as it is. I am quite surprised about this as I thought he would have increased the omeprazole but he said he didn’t think it would make a difference. We have got a follow up appointment in three months which basically seems like a lifetime away and I’m not happy with this. I know that the drugs need to be reviewed regularly in view of weight gain, does anyone know what is the maximum dose per kg of omeprazole is because I think he’s on the lowest dose (he now weighs 21lb 13oz). I don’t know what to do next, I think I would like to get a second opinion, preferably go private if possible (need to discuss with other half) to see a paed gastro specialist who knows more about the symptoms and various medications etc plus try a hypoallergenic formula if I can get Daniel to drink one. I’ve not had much success with bottle feeding thus far (currently breastfeeding) as every attempt I have made at getting him to drink from a bottle has failed miserably, I’ve tried different bottles and different formulas which he won’t drink so I don’t hold out much hope on that score if the hypoallergenic formulas taste and smell as bad as I’ve heard.

Any suggestions/advice would be welcome.

There's definately room for increasing the omeprazole dose and can't understand why he's decided to increase the domperidone only. My baby weighs about 16 lb now but has been on 20 mgr of omeprazole for ages- can't remember her exact weight when they prescribed the current dose.
I haven't gone private myself but quite a number of people here have.
There's a paed especialised in GI in London called Dr. Thomson, his website is www.paediatricgastroenterologist.co.uk (it also has information about the different drugs used for the treatment of reflux and of the doses) some babies here have been seen by him with great results, so hopefully they'll be able to give you more information about him and other private paeds in London.
There's also a Dr. Shah in Great Ormond Street who sees patients privately- Kelly can give you more information about him.

Thanks for the info regarding Dr Thomson and Dr Shah. I am considering whether to get a second opinion from a specialist, even if there's no improvement in Daniel's condition, at least then I'll know we exhausted every avenue.

Really hope things improve for Daniel soon. If you can afford it then I would definately go private and get a second opinion.

Hi Lisa,
Dr Shah is great, £240 and well worth every penny. Callum is like a different baby all together now. You just need to get a gp referal for this, can give you the info and numbers if you want them.
